HOME FROM HOSPITAL

If you require support with getting to your appointments, you can always rely on our professional service. We will pick you up from your door, and drop you off when needed, with your permission, we can accompany you in to your meeting and go over any information you are unsure of.

​Following discharge from hospital following illness, accident or medical procedure, our care staff are here to meet your needs, while still promoting your independence.

    JOIN OUR TEAM

    As a home care provider, our community staff are generally mobile, travelling from client to client to deliver care and support. The roles are varied and rewarding, working in the community providing care and support for people wishing to remain as independent as possible in their own home, in consideration of varying levels of support.

    Duties will typically include the following:

  • Help rising and preparing for bed

    HOME CARE PERSONNEL (COMMUNITY CARER)

    • Delivering care and support to our clients in their own homes in Telford
    • Assisting with personal care, mobility Issues, overseeing medication, meal
      preparation light domestic duties etc.
    • Experience is preferable however full training is provided for those new to care
    • As this is a mobile role so own transport is required
    • Available hours to work can be flexible but there is a need to be able to work some evenings and alternative weekends.
    • Care duties include attending to the needs of the client in accordance with the bespoke care plan and risk assessments, promoting independence wherever possible respecting the service users’ dignity and privacy.
    • At all times observing safe working practices, Interact with clients offering support, encouragement and reassurance
    • Continence management, monitoring skin condition and pressure areas
    • Oversee and record medication
    • Record keeping will be clear, accurate and concise.

    SUPPORT WORKER (WAKING NIGHTS)

    • Providing care and support for clients throughout the night, attending to their needs.
    • Experience of this type of waking night care is essential to ensure the clients safety in all eventualities, shift duration and start/finish times vary dependant on client
      needs
    • Care duties include attending to the needs of the client in accordance with the bespoke care plan and risk assessments, promoting independence wherever
      possible respecting the service users’ dignity and privacy.
    • At all times observing safe working practices, Interact with client offering support, encouragement and reassurance
    • Assisting with general mobility, Personal care and hygiene Dressing and undressing
    • Preparation and serving of food and drinks, light domestic duties as required.
    • Continence management, monitoring skin condition and pressure areas
    • Oversee and record medication
    • Record keeping will be clear, accurate and concise.

    SUPPORT WORKER (DAY SHIFT)

    • Caring for clients we support for longer durations in the home and the community
    • Care duties include attending to the needs of the client in accordance with the bespoke care plan and risk assessments, promoting independence wherever possible respecting the service users’ dignity and privacy.
    • At all times observing safe working practices, Interact with client offering support, encouragement and reassurance
    • Assisting with general mobility, Personal care and hygiene Dressing and undressing
    • Preparation and serving of food and drinks, light domestic duties as required.
    • Continence management, monitoring skin condition and pressure areas
    • Oversee and record medication
    • Record keeping will be clear, accurate and concise.
